Patient Information: New Patients

Your first visit to Professionals in Dentistry establishes a vital foundation for our relationship with you.  During the first visit, we make sure to obtain important background information, like your medical history as well as your previous dental history.  To understand what to expect for your first visit to our practice, please read through this page. There is also background information about our committed staff and our first visit procedures. 

Please assist us by providing the following information at the time of your first visit:

  • A list of your current medical prescriptions
  • Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
  • If you have dental insurance, bring copy of the front and back of your dental insurance card.  This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit. In the case of blended families, the parent who brings the child for treatment will be financially responsible regardless of insurance involvement and divorce declarations.

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.  We follow the American Dental Association guidelines for cavity detection x-rays and full mouth diagnostic panoramic x-ray.  It is recommended that you have cavity detection x-rays every 12 months and the panoramic diagnostic radiograph every three years.
