It is important to be well informed regarding the extent of your insurance coverage. Every insurance carrier and employer is unique in the benefits they offer their employees; our staff does not know what services are covered in your plan. Please refer to your benefits manual and/or your insurance website to be familiar with you particular benefits. It is our stance that you are responsible to know your insurance benefits.
Whether or not the treatment you require is a covered benefit depends upon the contract your employer purchased with the insurance company. You can expect your insurance company to reimburse you for a portion of our fee.
We want to help you receive the maximum insurance reimbursement to which you are entitled. As a convenience to you, we will help you process your insurance claims. We will also gladly provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services provided. At all times, you can be confident that we will always provide you with our best services without regard to the limitations imposed by your insurance coverage.
Our office accepts assignments of benefits for many insurance companies and participates as a plan provider for two companies. This permits us to file your insurance claim as a courtesy to you, and allows the insurance company to render payment directly to our office. However, not all insurance companies will accept such assignments of benefits. In these cases, you will be required to make full payment at time of services; we will still file the claims on your behalf. Our office provides insurance estimates only; your actual indemnity may be less. The patient is responsible for the payment of treatment regardless of insurance involvement.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
